How to Apply Local Guidelines Reimbursement


TYPES OF GRANTS

Standard LCC grants are awarded to groups and individuals. PASS grants fund cultural field trips for children.

HOW TO APPLY

Applications are available at The Marshfield Town Hall lobby, Ventress Library information desk and the Clift Rodgers Library entry. Applications must be complete and typed, including budget and signature sections. Handwritten applications (partial or total) will be ineligible.

You may also obtain Standard or PASS grant applications on-line through Massachusetts Cultural Council's website.

Applications must be mailed via the United States Postal Service and postmarked by October 15, 2002.

Submit only one copy of your application. Be sure to keep a copy for yourself.

Applications should be mailed to: Marshfield Cultural Council, P.O. Box 1052, Marshfield, MA 02050; applications sent to any other address will be ineligible.

Slides, photographs, or tapes will not be returned unless accompanied by a self-addressed, stamped envelope.

LOCAL GUIDELINES

Performers must have a venue (sponsor/site) or partnership.

Multiple grant applicants should prioritize projects.

In accordance with state guidelines, the Marshfield Cultural Council may choose not to fund repeat applicants.

Notify the council of the date, time and place of your event as soon as it is known.

REIMBURSEMENT

Funds granted are paid on a reimbursement basis only following project completion.

Projects must be completed within one calendar year from the date of approval.

Grantees must submit a Final Report when requesting reimbursement. Those who fail to do so may be ineligible to receive subsequent funding. You must provide supporting documentation (i.e. copy of program, receipts for payment, verification from sponsor).

Grant recipients are required to acknowledge the Marshfield Cultural Council and the Massachusetts Cultural Council as funding sources. Future funding may be jeopardized if you fail to do so.

Complete and include a Form W-9 unless you are certain the town has one on file for you.
